Welcome to VerityCare Clinic (“VerityCare”, “we”, “us”, “our”). By accessing or using our website (www.veritycare.com), patient portal, telemedicine platform, or by scheduling and receiving healthcare services, you agree to be bound by these Terms of Service (“Terms”). If you do not agree, please do not use our services. These Terms incorporate our Privacy Policy and any additional posted guidelines.

VerityCare provides primary care, specialty consultations, wellness exams, chronic disease management, telehealth, and related clinical services. No doctor-patient relationship is established solely by using this website; a formal relationship begins when you schedule an appointment and receive evaluation from a licensed provider. All medical decisions remain the responsibility of your treating clinician. In case of emergency, call 911 or go to the nearest emergency department — do not rely on electronic communication.
You must be at least 18 years old, or have parent/legal guardian consent, to use our online services and create a patient portal account. You agree to provide accurate, complete, and current information during registration. You are responsible for maintaining the confidentiality of your portal login credentials and for all activities under your account. Notify us immediately of any unauthorized access. VerityCare reserves the right to suspend or terminate accounts that violate these Terms.
2.1. Patient Portal (MyVerity)
Our portal offers secure messaging, prescription refill requests, appointment scheduling, and access to medical records. You agree not to use the portal for urgent medical matters. We monitor portal activity for security and quality assurance. You may request deactivation of your account at any time.
Appointments can be scheduled via phone, website, or patient portal. We require at least 4 hours' notice for cancellation of in-person or telehealth visits to avoid a fee. Late cancellations (less than 4 hours) and no-shows may incur a $35 fee. For established patients, three or more no-shows within 12 months may result in discharge from the practice. We understand emergencies happen — please contact us as soon as possible. For annual wellness and complex visits, a 24-hour notice is appreciated.
VerityCare offers real-time video consultations. By using telehealth, you acknowledge that:
- Telehealth is not suitable for all conditions (e.g., chest pain, severe trauma, suicidal ideation).
- Technical failures (internet, video, audio) may interrupt the session; in such cases we will attempt to reconnect or reschedule.
- You are responsible for providing a private, quiet environment. We recommend you test your device/camera before the visit.
- We may record the telehealth session only with your explicit written consent; otherwise, recordings are prohibited.
Telehealth visits are billed similarly to in-office visits, and insurance coverage varies. You will receive a good-faith estimate prior to service (No Surprises Act).
Our service prices are published on the About page but may change. We accept cash, credit cards, HSA/FSA, and most major insurance plans (including Medicare and Medicaid). Any balance not covered by insurance is your responsibility. Payment is due at the time of service unless prior arrangements are made. For recurring monthly care management plans, you authorize automatic charges to the card on file. Delinquent accounts may be sent to collections after 90 days of non-payment. We offer financial assistance for qualifying low-income patients — ask our billing department.
Prescriptions are issued at the sole discretion of your VerityCare provider. Controlled substances are subject to state and federal regulations and may require periodic in-person visits. We do not prescribe controlled substances via telehealth for new patients without an initial in-person evaluation (as required by federal law). Refill requests should be submitted through the patient portal; please allow 2 business days for processing. We reserve the right to deny refills for medical or safety reasons.
All content on this website — including text, graphics, logos, images, and software — is the property of VerityCare or its licensors and is protected by copyright and trademark laws. You may not reproduce, distribute, modify, or create derivative works without prior written permission. You are granted a limited, non-exclusive license to access and use the site for personal, non-commercial purposes (e.g., scheduling appointments, reading health information). Unauthorized scraping, reverse engineering, or automated access is prohibited.
When interacting with our website, patient portal, or social media channels, you agree not to:
- Post false, defamatory, abusive, or harassing content.
- Upload viruses, malware, or other harmful code.
- Attempt to gain unauthorized access to our systems or other users' accounts.
- Interfere with the operation of our services (e.g., denial-of-service attacks).
- Use our services to violate any applicable law (including HIPAA) or medical ethics.
Violation may result in immediate termination of access and legal action.
Our website may link to external websites (e.g., lab portals, pharmacy partners, government resources). VerityCare does not endorse or assume responsibility for the content, privacy practices, or terms of any third-party sites. Your interactions with those sites are governed by their own terms.
VerityCare provides services “as is” and “as available” without warranties of any kind, either express or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, or non-infringement. We do not warrant that the website will be uninterrupted, error-free, or free of viruses. Medical information on this site is for educational purposes; always consult a qualified professional for personal medical advice.
To the fullest extent permitted by Texas law, VerityCare and its physicians, employees, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our services or website, even if advised of the possibility of such damages. Our total aggregate liability for any claim arising from these Terms or the provision of services shall not exceed the amount you paid to VerityCare in the past six months. This limitation does not apply to claims of gross negligence, willful misconduct, or personal injury caused by our direct negligence.
You agree to indemnify, defend, and hold harmless VerityCare, its officers, clinicians, and employees from any claims, liabilities, damages, losses, and expenses (including reasonable attorney’s fees) arising from: (a) your violation of these Terms; (b) your misuse of our services; (c) your violation of applicable laws or rights of a third party; or (d) any content you submit through our platforms.
We may suspend or terminate your access to our website, portal, or clinical services at our sole discretion, without notice, for conduct that violates these Terms, endangers staff/patients, or for non-payment. If we terminate your patient relationship for cause, we will provide a 30-day notice to facilitate transfer of your medical records to another provider, except in cases of abusive or violent behavior (immediate termination).
Informal resolution first: Most disputes can be resolved by contacting our patient relations team at (512) 555-1234. If unresolved, any dispute arising from these Terms or your care (except small claims court matters) shall be resolved by binding arbitration in Austin, Texas, under the rules of the American Health Law Association. You waive the right to participate in class actions or class arbitrations. Judgment on the arbitration award may be entered in any court of competent jurisdiction. This provision does not apply to emergency injunctive relief or claims regarding intellectual property.
These Terms and any disputes shall be governed by the laws of the State of Texas, without regard to its conflict of laws principles. The exclusive venue for any litigation not subject to arbitration shall be the state or federal courts located in Travis County, Texas.
VerityCare reserves the right to revise these Terms at any time. Material changes will be communicated via email or website notice at least 30 days in advance. Your continued use of our services after the effective date constitutes acceptance of the updated Terms. The “Last Revised” date indicates when changes were made.
If any provision of these Terms is found unenforceable, the remaining provisions shall remain in full force. No waiver of any term shall be deemed a further or continuing waiver of such term. These Terms constitute the entire agreement between you and VerityCare regarding the use of our website and services, superseding any prior agreements. Section headings are for convenience only.
